2007 Annual COER is now closed
To complete the 2007 Annual COER, please complete the prescribed Manual COER forms and submit to OCCO through your Commissioned Corps Liaison. The Manual COER forms consist of PHS Form 838, Attachment 1, and Attachment 2. The Manual COER forms are located under the "Performance Section," of the following Web link: http://dcp.psc.gov/cc_forms.asp. Please NOTE, original signatures are required and fax copies of COERs will not be processed.
