All officers are required to have a physical examination, signed by a medical provider within the last 5 years. They must also have a completed form DD-2807-1 “Report of Medical History” accompanied by a self-signed “Disclosure Statement” sent to Medical Affairs Branch (MAB) and current within one year prior to the due date on the Promotion Checklist.
If eligible for any type of permanent promotion (competitive or non-competitive), OCCO strongly urges that the following actions be completed:
1. Logon to the secure area of the CCMIS Web site.

2. Review the status of 5 year annual physical and ensure that it is and will be current within 5 years as of the due date on the Promotion Checklist. If it is, or will be outdated, please print out the Instruction Letter, Report of Medical Exam (5 Year Physical) and Report of Dental Exam and take the forms to the appropriate providers to have completed. Once accomplished, MAIL (DO NOT FAX) a copy to the MAB at the address located below.

3. Review the status of “Report of Medical History” (DD Form 2807-1), and ensure that it is current within one year prior to the due date on the Promotion Checklist. If the medical history form is, or will be outdated, print both the Instruction Letter and DD Form 2807-1. Follow these instructions:
  • Write ‘USPHS’ in Section 6.a.
  • Note the purpose in Section 6.c. as ‘PROMOTION’
  • Complete this form in its entirety and date
  • This form DOES NOT require a medical provider’s signature; this is self reported  information only
  • Ensure the ‘Disclosure Statement’ is completed, signed and dated and sent in accompanying the DD Form 2807-1
  • Mail to MAB at the address below
Promotion board members do not consider this as part of their review of the officer’s record and they do not have access to this information. This requirement will be applied after the category promotion boards meet and prior to permanent promotion orders being issued. Failure to have these forms updated with MAB will result in the removal of the officer’s name from the permanent grade promotion list during the post board administrative review.
Please note: Based on the review the information provided either on the Physical Exam or Report of Medical History, the Medical Affairs Branch will notify the officer of any additional requirements.
