USPHS Commissioned Corps Customer Service Satisfaction Survey

You are invited to participate in the U.S. Public Health Service Commissioned Corps Customer Service Satisfaction Survey beginning on Monday, March 27, 2006 and ending on Tuesday, April 25, 2006. Officers will receive an e-mail from the survey contractor, Anteon Corporation, who will provide the online survey location and unique access credentials for each officer to complete the survey.

The intention of the survey is to assess the customer service provided by the Commissioned Corps’ administrative and management organizations for the active-duty force. The survey asks a variety of questions that will help assess the current performance, identify areas for improvement, and develop benchmarks to measure future progress in each of the following organizations:

Office of Commissioned Corps Operations (OCCO)
Office of Commissioned Corps Force Management (OCCFM)
Commissioned Corps Systems Branch (CCSB)
Compensation Branch (CB)
Medical Affairs Branch (MAB)
The survey will take approximately 10 to 20 minutes to complete. Your survey responses will be coded and remain strictly confidential. Anteon will analyze the responses and will only report data to the Corps in the aggregate.

If you have any questions at any time about completing the survey, you may contact Dietrich Wiegmann at 202-756-5394 or by e-mail at dwiegmann@anteon.com.

Your participation in the survey is very important to the organizations being assessed. Thank you in advance for your time and participation.
