The Dental Professional Advisory Committee (DePAC) is soliciting self-nominations for a 3-year commitment beginning January 2006. |

Please follow the instructions listed below carefully, and send all documents electronically to swisner@bop.gov
by the close of business on 1 September 2005. |

- Complete the self-nomination form which is available at http://www.phs-dental.org/depac/self_nom.doc and have your supervisor sign-off.
- Send current curriculum vitae.
- Send cover letter with a statement expressing your reasons for serving on the DePAC.
