| Important Information Regarding the Tower Building PHS Commissioned Corps ID Card Issuing Office |

The Public Health Service Commissioned Corps identification (ID) card issuing office is located in the Tower Building, Suite 100, Plaza Level, 1101 Wootton Parkway, Rockville, MD. Our hours are 9 a.m. to 3 p.m. (Eastern time),
Monday through Friday. |

It costs $4.00 to park in the visitor’s lot at the Tower Building, whether your stay is for 15 minutes or 5
hours. Visitors must purchase a $4.00 token inside the building and use the token to exit the parking lot.
There is no one to take cash at the parking lot. Officers and/or dependents needing new ID cards or Defense
Enrollment Eligibility Reporting System (DEERS) services should plan accordingly when visiting the Tower Building. |
