Processing of Form PHS-1662, “Request for Personnel Action - Commissioned Officer”

Form PHS-1662 is used to request all types of personnel actions, (excluding separations and retirements).
The following are some of the types of personnel actions that are considered as calls to active duty (CAD): |

- Extended Active Duty
- Conversion from Civil Service
- Program Limited Tour
- CAD for Training - (1) Junior Commissioned Officer Student Training and Extern Program (COSTEP), and (2) Senior COSTEP

Subchapter CC23.6, INSTRUCTION 2, Section D.1.b. and c. provides that: (1) forms PHS-1662 requiring no
travel must be received by the Division of Commissioned Corps Assignments (DCCA) no later than 10 calendar
days from the effective date of the form, and (2) forms PHS-1662, where travel is involved (regardless of
the number of travel days), must be received by DCCA no later than 20 calendar days before the effective
date of the form. |

The Office of Commissioned Corps Operations (OCCO) seeks to provide excellent customer service to all of
our customers. To assure the fair and equitable processing of requests, OCCO will be following this
policy and the suspense time periods noted above. We ask that managers honor this requirement,
particularly when requesting CAD orders and Permanent Change of Station orders. OCCO staff will work
closely with the Commissioned Corps Liaison in adjusting the effective dates of forms PHS-1662 that do
not meet the 10- or 20-day time frame. |
