Electronic Health Information - Update

TO: Officers of the Commissioned Corps of the U.S. Public Health Service (Corps)

Beginning on 1 June 2010, all immunizations information collected, verified, and reported by the Medical Affairs Branch (MAB) will be utilized as the sole source of information to determine numerous components of medical readiness, promotion eligibility, etc.

As Corps officers, please review your information by accessing your immunization details at http://dcp.psc.gov to make certain all of your immunizations data have been captured electronically. If you find a discrepancy, or you have additional information to submit, please mail or fax the document to the MAB Fax number at 301-594-3299. Allow approximately 1 to 2 weeks from date of receipt for the item to be collected, reviewed, and electronically documented. Please ensure that your full name and PHS Serial Number appear on each immunization document.

We are working diligently towards fully electronic officer medical records and look forward to offering this product as soon as possible.
