Pharmacist Responder of the Year Award
Deadline Date: 7 December 2008

This award recognizes a pharmacist (officer of the Commissioned Corps of the U.S. Public Health Service (Corps), Department of Health and Human Services civil service employee, or temporary Federalized responder) that has provided outstanding emergency preparedness, disaster response, and contributions to national or international public health threats within the past 12 months.

Details of the award criteria and nomination packets are available on the Award Section of the PharmPAC Web site – http://www.hhs.gov/pharmacy/award2.html

Please take the time to nominate a deserving pharmacist (Corps, civil service, or temporary Federalized responder) for this award. If a Corps officer is the recipient of the Pharmacist Responder of the Year Award, the officer’s name is forwarded to the Office of Force Readiness and Deployment (OFRD) as a nominee for the Commissioned Corps Responder of the Year Award. In the event that the PharmPAC selects a civil servant or a temporary Federalized responder as the recipient of the award, then the Corps officer that received the highest ranking from the PharmPAC is forwarded to OFRD.

Please submit nomination and appropriate documentation to CDR Mary Kremzner electronically at mary.kremzner@fda.hhs.gov by close of business, Monday, 7 December 2008.
