                    | Commissioned Officer’s Effectiveness
                            Report (COER) |

                  | The COER is located in the blue section of the eOPF. ALL officers (regardless
                      of date of entry onto active duty) who are eligible for competitive promotion 
                          are required to have
                      either an Annual or a manual interim
                      COER in the eOPF. The manual interim COER is ONLY completed if called to duty after July 1 of the current COER year
                      so the Promotion Boards will have a means of assessing recent performance. All officers
                      on duty prior to July 1 of the current COER year must have an Annual COER in their
                      eOPF (Note: A manual interim COER will 
                          NOT be accepted if an Annual COER is required). To complete
                      a manual interim COER, go to the
                          CCMIS Web site, select “Services,” then "Official
                      Forms," and scroll down to “Performance.” Print out and complete form PHS-838, Attachment
                      I and Attachment II. All manual COERs (such as the Interim COER) must be processed
                          through the HHS OPDIV/STAFFDIV or non-HHS organization channels, Commissioned Corps Liaisons, and be received in OCCO no later
                              than the deadline on the 
                                  Promotion Checklist to ensure availability for the Promotion Boards.
                      They will NOT be accepted directly
                          from officers or through the eOPF fax numbers. |

                    | Officers are encouraged to access their latest COER in the eOPF and review each
                        page to ensure that the document has been satisfactorily archived. Officers
                            should also ensure the last 5 years (as applicable) of COERs are on file in their eOPF.
                        Missing COERs within the last 5 years may result in adverse actions. Failure to
                        have all the required annual COERs on file will result in removal from the promotion
                        pool at the end of the boards. |
