It would be an honor to present the Colors at your event. If you are interested in requesting the Surgeon General’s Honor Guard, please fill out the request form (https://usphs.gov/docs/HonorGuardRequestForm.docx) and send it via email to SGHG@hhs.gov 45 days prior to the event.
