Effective 1 April 2013, Delta Dental of California was selected to serve as the contractor to administer the PHS Active Duty Dental Program. All active duty PHS officers have been automatically enrolled and no further action is required to participate. There are no premiums, co-pays or deductibles associated with the program.

Delta Dental has created a webpage solely dedicated towards serving active duty PHS officers, which can be found at: www.phsaddp.com. Here, you will find a list of all Delta Dental Premier Providers, answers to common questions and other pertinent programmatic information related to your dental care/benefits. Recently, a new Proof of Coverage I.D. card was added to the website. All officers should fill out the card, print it out and retain it to display program eligibility and participation. For your convenience, this card is also provided at the bottom of this communication.

Please remember, ONLY active duty PHS officers are eligible to participate in this program. Dependents of active duty PHS officers are eligible to enroll in the TRICARE Dental Program, which has been administered by MetLife since 1 May 2012. You can obtain information about this program by visiting http://www.metlife.com or calling 1-800-MET-LIFE to speak to an agent.

Additionally, PHS retirees and eligible dependents may enroll in the TRICARE Retiree Dental Program. Information about this program can be found at: http://www.trdp.org.
