Dear Civilian, Tribal, and Commissioned Corps Nurses,

The Federal Public Health Service Nursing (FPHSN) Strategic Plan Implementation Team and I, RADM Kerry Paige Nesseler, USPHS Chief Nurse Officer, would like to invite you to participate in a brief survey that will help illustrate Federal PHS nurses’ contributions to our FPHSN Strategic Plan, specifically in health promotion and prevention efforts.

The FPHSN Strategic Plan aligns with the goals of the U.S. Surgeon General’s 2011 Report, “National Prevention Strategy – America’s Plan for Better Health and Wellness” and outlines a nursing strategy to address three impact areas wherein nurses are best positioned to make an effect in the prevention activity areas of: 1) educating and empowering individuals, families and populations; 2) promoting the implementation of Federal Health programs, policy, regulation and research activities; and 3) encouraging participation in National Health Promotion Campaigns.

If you are a Federal PHS Nurse (HHS civil service nurses, tribal nurses, and all commissioned corps nurses), please complete this survey. The purpose of the survey is to identify your contributions to the following four priority health initiatives: 1) heart healthy; 2) healthy eating; 3) mental and emotional well-being; and 4) tobacco free living. You will be asked to rate your participation level on these priority health initiatives and will be given the opportunity to highlight your activities within your work and community (school, church, professional associations, family, self, etc).

Due to deployments and other activities in support of the Sandy Storm response, the survey has been extended to Friday, December 14, 2012. If you have already completed the survey—THANK YOU!!! If you have not had the opportunity to complete it yet….please consider completing the survey, We estimate that this survey will take approximately 20 minutes to complete. Please complete it in one sitting. If you log off before you complete the survey, you cannot go back into the same survey; you would have to begin again. Your responses will be anonymous. This survey has been approved through the Office of Human Subjects Research Protections at the National Institutes of Health (NIH) Clinical Center.
