The USPHS Commissioned Corps is committed to our mission to protect, promote, and advance the health and safety of the nation. Public Health Service officers are trained and equipped to provide crucial public health leadership, assist with policy development, advance innovation and science, provide essential care services and respond to national and global public health emergencies. Through our Active Duty Regular Corps, our Public Health Response Strike Team (PHERST) and our Ready Reserve Corps, we remain responsive and available to rapidly deploy in the service of health.

Healthcare Benefits and DEERS FAQ
Medical coverage
The officer needs to update their contact information in the Defense Enrollment Eligibility Reporting System (DEERS). The instructions can be found at: https://www.tricare.mil/deersActive duty officers will have coverage through TRICARE Prime or TRICARE Prime Remote, as determined by TRICARE.
Active duty family members can enroll in one of the Prime plans with their sponsor, or they may qualify for another plan if available. Enrollment changes outside of TRICARE Open Season https://www.tricare.mil/openseason are allowed following a Qualifying Life Event (QLE).
Please visit https://www.tricare.mil/LifeEvents to determine what constitutes a QLE. Refer to https://tricare.mil/plans> to see what plans are available to your family members.
Officers and family members should contact their TRICARE regional contractor (Humana or HealthNet) to have a new Primary Care Manager assigned.

Update your contact information on the Readiness Self Service Page in the Officer Secure Area of the CCMIS Website.Delta Dental:
Log into your Consumer Toolkit account at: https://www.ddfgptoolkits.com/ipWeb/appmanager/ct/desktop Create an account if you do not have one, Delta Dental sends notifications informing you that an Explanation of Benefits has been uploaded for your review.Family Member Dental Coverage:
Update your new contact information with the TRICARE Dental Program (TDP) contractor, United Concordia. To learn more about updating your family’s dental benefits, please visit: https://tricare.mil/CoveredServices/Dental/TDPGo to the CCMIS website, select Medical Affairs. Information on plans for active duty, family members and retirees is available under “Dental.”
Retirement is a Qualifying Life Event (QLE), you have 90 days after your retirement date to enroll in a health plan. Visit the TRICARE site for information: https://www.tricare.mil/LifeEvents/Retiring
If you are over age 65 and entitled to Medicare Part A, sign up for Medicare Part B before you retire. Fill out Section A of CMS-L564 form. Scan the form and send it to the CCHQ Medical Branch mailbox in eCMCS or PHSMACCHQ@hhs.gov. Medical Affairs will complete Section B and return the form to you. You need to include it with CMS-40B, Application for Enrollment in Medicare when you submit your application to the Social Security office.
Active duty officers with TRICARE Prime who get their routine care through a Military Treatment Facility (MTF) should go the MTF for emergency and urgent care. Active duty officers with Tricare Prime Remote can go to any Tricare covered emergency department or urgent care.

If you become ill while away from home, we strongly recommend that you call the Tricare 24/7 Nurse Advice Line at 1-800-TRICARE (874-2273) to ensure you get care and avoid out of pocket costs. There are different rules for retirees and family members. Information on urgent care is available at: https://tricare.mil/CoveredServices/IsItCovered/UrgentCare
