  eOPF Corrections	Review electronic Official Personnel Folder (eOPF) and submit requests for corrections to phsopffix@hhs.gov prior to the deadline.	February 17, 2017
  Review Benchmarks	Review Category Benchmarks to assist in preparing for promotion.	ASAP
  CV Format	Check with your Professional Advisory Committee (PAC) for category guidance on formatting your CV (and CV Cover Page). Only use your PAC’s CV format.	ASAP
  CV and Cover Page Submission	Submit current CV and CV Cover Page (as one document) to your eOPF using the eDOC-U upload system in the Secure Area on CCMIS. NOTE: Be sure to select the correct document using the drop-down menu and enter the correct date.	February 17, 2017
  COERs**	Ensure that you are not missing any Commissioned Officer’s Effectiveness Reports (COERs) in your eOPF. Review COERs to ensure that all pages are present and legible. For New Officers: we recommend that you have a recent COER in your eOPF, even if not required. For additional assistance with COER-related matters, first visit the COER Information Page, and then contact your CC Liaison if needed.	February 1, 2017
  Officer's Statement	Submit a 2017 Officer's Statement (OS) to your eOPF using the eDOC-U upload system in your secure area eOPF. NOTE: Only the 2017 form will be accepted.	February 17, 2017
  Reviewing Official's Statement	Ensure that the 2017 Reviewing Official's Statement (ROS) has been submitted to and received by DCCPR via your CC Liaison prior to the deadline. NOTE: Only the 2017 form will be accepted, and must be submitted through the Agency Liaison.	January 31, 2017
  Required License/ Certification**	If required, ensure that your PIR reflects your current (not expired), valid Professional License or Certification at all times and at least by the checklist deadline. The actual license does not need to display in your eOPF by the deadline.   Force Readiness**	Ensure that you maintain all Basic Force Readiness Requirements. You may check your most recent quarterly status in Direct Access. Visit the Readiness Checklist for documents and submission requirements. Force Readiness standards will be verified as of both dates listed.	February 17, 2017 and March 31, 2017
  Medical Requirements for Permanent Promotion**	If eligible for a permanent grade promotion, in addition to all other requirements listed above, ensure that you have the following on file with Medical Affairs (MA): A "Report of Medical History" (form DD-2807-1) and accompanying self-signed "Disclosure Statement" current within one year of the deadline (received by MA May 1, 2016 to April 30, 2017) A current 5-Year Physical Examination (expires May 1, 2017 or later) Refer to the Medical Requirements for Permanent Promotion in the Table of Contents for a detailed explanation of requirements. Check the Secure Area of the CCMIS website to determine the dates of the last documents on file with MA.	As of April 30, 2017

  ** IMPORTANT!!! These criteria will be reviewed during DCCPR’s post-Board administrative process (normally conducted between April and June). Failure to comply with any of these criteria will result in the officer not being promoted.

  Promotion Instructions are available on the Commissioned Corps Issuance System (CCIS).

  • Select "Commissioned Corps Issuance System" from the link bar at the top of this page.
  • Click on "Table of Contents."
  • Click on the "+" next to "Book 3: Personnel Management."
  • Click on Chapter 3: Promotions and Reductions.
  • Click the "+" next to the topic of interest related to promotions. The section title will turn blue.
  • Double click the blue section title and refer to the policy governing this process.
