  Officer’s Statement (OS)

  The OS is a 1-page document that provides you the opportunity to summarize:

  • Your support of U.S. Public Health Service (USPHS) Commissioned Corps activities
  • Your commitment to visibility as an officer
  • Your vision and expectations of a career in the USPHS Commissioned Corps, including commitment to the USPHS mission

  Directions:

  1. Download the 2025 OS Form.
  2. Fill out the administrative data and answer the questions.
  3. Sign and date the form.
  4. Upload the form to your eOPF no later than the due date of the Promotion Checklist.
  5. Ensure your document is viewable in both your eOPF (indexed into the COER section) and OPPVS.

  Note: Only the current year’s OS Form is accepted for promotion consideration and for inclusion into the eOPF and OPPVS. No other versions will be accepted. Additionally, the Promotion Board will only have access to the current year’s OS; they will not see OSs from previous years. Therefore, you must submit a new OS every year you are eligible.

  General Guidance

  • Statements should describe not just what organizations you are a member of, but also what contributions you make to those committees or groups
  • Show accomplishments and impact, not just job descriptions or membership
  • OS should not relist all the items on the CV
  • Do not add items that are required (wearing uniforms, basic readiness, etc.)
  • Use data and impact statements
  • Make it quickly and easily read
  • You do not need to fill in all the space on the sheet; often, “less is more”
  • Leave a blank space between each of the three sections
  • Use of bullets is recommended

  Examples

  Your support of USPHS Commissioned Corps activities

  • Oversaw ordering and distribution of XX vaccines, administration of XX vaccines at XX facilities across XX states, serving XX people
  • Led the successful implementation of local COVID-19 emergency response plans, which quelled a facility outbreak of XX patients

  Your commitment to visibility as an officer

  • Presented about USPHS, in uniform, to XX schools of nursing, high schools, vocational schools, and career fairs
  • Led a qualifying PHS Athletics Event with XX officers and XX civilians

  Your vision and expectations of a career in the USPHS Commissioned Corps, including commitment to the USPHS mission

  • Serve in XX agency and continuously seek opportunities to improve my clinical and technical skills
  • Serve vulnerable populations and provide excellence in public health practices

  2025 OS Form

  Promotion Instructions are available on the Commissioned Corps Issuance System (CCIS).

  • Select "Commissioned Corps Issuance System" from the link bar at the top of this page.
  • Click on "Table of Contents."
  • Click on the "+" next to "Book 3: Personnel Management."
  • Click on Chapter 3: Promotions and Reductions.
  • Click the "+" next to the topic of interest related to promotions. The section title will turn blue.
  • Double click the blue section title and refer to the policy governing this process.
