As Public Health Service Officers and America’s Health Responders, we stand ready to preserve public health and national security during national or global health emergencies. Our mission is to be out the door as soon as requested, as this is an “all hands on deck” situation. All Corps officers were placed on involuntary deploy or "alert" status effective Wednesday, March 25, 2020, irrespective of the 'on-call month' status. Under Commissioned Corps Directive 121.02, "Deployment and Readiness," as directed by Assistant Secretary for Health, Commissioned Corps Headquarters will deploy Commissioned Corps officers assigned to HHS OpDivs/StaffDivs, either voluntarily or involuntarily and without supervisory approval, who are not considered mission critical, for deployments, under the current National Emergency. Mission critical requests sent to CCHQ are not final until the request is approved by the CCHQ Director as per the POM 821.76 ”Deployment Procedures”. An officer with a non-HHS organization may be deployed in accordance with the Memorandum of Agreement/Understanding between the Public Health Service Commissioned Corps and the non-HHS organization.

Periodic Health Update (PHU) 2021
The U.S. Public Health Service (USPHS) Commissioned Corps continues to require officers to undergo annual periodic health updates in order to better protect the health of officers and meet mission priorities. Maintaining this requirement is also vital to assuring your continued access and continuity of services at military treatment facilities related to PHUs.
Therefore, with some allowances for the COVID-19 pandemic and the burdens it imposes, the PHU requirements will resume in 2021 starting with officers whose birth month is January. All officers will be required to complete and submit a PHU in their 2021 birth month period (from the first day of the month prior to their birth month until the last day of the month after their birth month). The only exception in 2021, will be officers with a January birth month. They may submit their PHUs as early as December 2020 but have until March 31, 2021 to formally submit. Lastly, some officers who entered the USPHS Commissioned Corps in 2020 may not be required to submit a PHU until 2022. Medical Affairs Branch (MAB) has already directly contacted those individuals.
The requirements for the 2021 PHU include seven components:
- Medical History: Self-reported, and given to the officer’s medical provider for review, verification, and signature (form DD-2807-1)
- Physical Examination: In-person examination based on the officer’s health risk factors, as determined by the officer’s provider. Minimal requirements are the officer’s vital signs, weight, height, and, if necessary body measurements (“taping”) to determine estimated percent body fat form DD-2808) [Cannot be opened in Chrome] and (form PHS-7044-1), if needed
- Behavioral Health Survey: Self-reported and given to the officer’s provider for review and signature. The behavioral health survey will facilitate a discussion with the provider about the mental well-being of the officer (form PHS-7083).
- Dental examination: done within the six (6) months prior to the PHU submission (form DD-2813) [Cannot be opened in Chrome]
- Additional tests: Screening tests as recommended by the United States Preventive Services Task Force (USPSTF) (e.g. cervical, breast and colorectal cancer screening) and any other testing recommended by the officer’s provider based on the officer’s age, medical, family, and deployment history, and the officer’s occupational and environmental exposures and risks
- Disclosure Statement (form): signed by the officer
- 2021 PHU Submission Checklist (form): completed and signed by the officer to attest to what components of the PHU are being submitted and why.
We recognize that officers may still have difficulty accessing their healthcare providers due to the COVID-19 pandemic. For 2021 only, if after a Public Health Service officer has made a reasonable effort to get in-person appointments without success, MAB will accept a PHU with the following components:
- The results of a “virtual” physical examination which contains a provider verified history,
- A completed behavioral health survey signed by your provider, and
- A report of a dental examination done within 12 months of the PHU submission or no dental examination, if you have been unable to schedule an appointment with your dental provider during the past year.
For 2021 only, you will be asked to complete and sign a checklist indicating which components of the PHU you are submitting and, if necessary, why you are not submitting a complete PHU. This will ensure that MAB does not reject your submission when you were unable to complete one or more components because of the pandemic.
Officers should provide all necessary forms and a copy of the PHU Instructions for Providers and Instructions for Completing DD-2807-1, 2808, 2813 to their medical providers prior to or at the time of their appointment. They should also take a copy of the dental form, DD-2813, to their dentist at the time of their appointment.
Officers should review the PHU FAQs to determine when their first PHU is due according to the last physical exam they submitted. Officers should take all necessary forms and a copy of the PHU Instructions for Providers to their medical providers at the time of their appointment. Officers should also take the following to their appointment - PHU Instructions for Completing DD-2807-1, 2808, 2813. They should also take a copy of the dental form, DD-2813, to their dentist at the time of their appointment.
All forms and provider instructions are included in a single PDF file, “PHU Appointment Documents,” which can be found on the CCMIS website under Forms/Medical.
HOW TO SUBMIT THE PHU
Officers should scan all documents and test results into a SINGLE PDF and submit them using the medical section of eDOC-U with the document name, “Periodic Health Update.”
